We deem digital integration in support of the elderly at the core of tackling demographic aging; technology can offer improved unique tools to guarantee continuous assistance to those who need it, both physically and remotely. We designed the INSIEME platform for Noon Care to offer an innovative teleassistance solution and support the new model of chronic patient care called Smart4, created by the Genera cooperative. Thanks to INSIEME, social enterprises can include telecare in their patient care offer to ensure total continuity of essential activities at home, even when contact is restricted. INSIEME is quickly activated by installing the dedicated app on any Android tablet and is designed to schedule video calls, including group calls, or set daily tasks and automatic reminders. The platform attempts to fulfill the need for an innovative support model for the most vulnerable and their families; we intend to expand the assistance scenarios and round-the-clock contact with social workers while putting forth a fresh way of keeping in touch with the loved ones who are unfamiliar with digital technology. It's a people-friendly application addressing the needs of all parties involved.
